Another program for incompatible live donor and recipient is a list exchange. An incompatible living donor provides a kidney to the transplant hospital’s deceased donor waiting list. In exchange, the recipient moves to the top of the region’s waiting list for their ABO blood group. To be eligible for the list exchange, it must be the recipient’s first deceased donor transplant. The recipient must be on dialysis and unsensitized (not have a high level of antibodies in their blood so that it would be difficult to match a kidney). The living donor and recipient must be on the paired exchange list for 90 days before moving to a list exchange.
