Parathyroid hormone (PTH) is an 84-amino acid single chain peptide. The amino acid portion PTH (1-34) is critical for its biologic action. It is secreted as preproparathyroid hormone (115 aa) → Proparathyroid hormone (90 aa) (aa, amino acid).
Actions of PTH
PTH Increases rate of dissolution of bone mineral
Renal action: ↑ reabsorption of Ca (distal tubule)
Inhibition of phosphate transport (proximal tubule)
Stimulation of 25(OH)D 1α-hydroxylase Inhibits 24-hydroxylase
Receptors for PTH PTH1R Responds to PTH and PTHrP
PTH2R Responds to PTH only
PTH-related peptide (PTHrP) is secreted by some squamous cell tumors.
It is also secreted by mammary tissue (secreted into milk).
Decreased PTH release and synthesis occurs by:
• Calcium (ionized form) acting through CaSR (hypercalcemia)
• 1,25(OH)2D
• Hypermagnesemia
• Severe hypomagnesemia.
