C. Peripheral slide reveals the presence of two populations of blasts: one with large cells with high nuclear to cytoplasmic ratio, dispersed chromatin, and prominent nucleoli, and another population with small cells with nucleoli and less dispersed chromatin. Flow cytometry reveals expres sion of both myeloid and lymphoid markers. This is characteristic of MPAL, a condition that com prises less than 5% of acute leukemia cases and is usually associated with a poor prognosis (Leukemia. 2010;24(11):1844, Leukemia. 2010;24(7):1392, Blood. 2011;117(11):3163).
